Incident Reporting

All accidents and incidents, including injury, fire, dangerous occurrences and near misses, must be reported to us so we can ensure incidents are investigated appropriately and lessons learned to prevent a similar incident occurring.

To report an incident, please complete a form using our GRaCE system as soon as possible. In the event of a serious incident (e.g., fire or significant injury), please call (0191 33) 46453, or email us without delay.
